Admission
Individuals must register with the Fraser Health Hospice Palliative Care Program through family physicians, social workers or other health professionals. They can be admitted into hospice if they:

  • agree with the goal of hospice care
  • have a life expectancy of months
  • cannot be cared for at home
  • require symptom management but not hospital care

The patients are encouraged to agree with the goals of hospice care where there is limited medical intervention and patient comfort and support are paramount.

A home care nurse can assist the patient and their family with the choices available to them, including when is the best time to come to hospice. We encourage patients and their families to arrange for a tour of the hospice prior to admission if they wish.

Fees for service
There is a $30.90 per day fee (subject to change annually) for hospice stay however no person will be denied hospice care if they are unable to pay. An invoice will be sent and payments are made to Crossroads Hospice Society. Payments can be made by cheque and/or Visa/MasterCard.

Family Meetings
Shortly after admission patients will meet with Hospice staff who will provide an orientation to the facility, explanation of the process of dying, and explore the expectations and wishes of patients and families during this difficult time. Family meetings play an important part of the process. The patient makes the decisions on their care but family needs must be considered as well. Cultural, social, and spiritual considerations can play a role in planning someone's final days as well as finding opportunities for individuals to express themselves as required.

Bereavement Support
We are committed to the ongoing provision of emotional support to family and friends following the death of a loved one. Practicing a philosophy of 'companioning the bereaved', we regularly extend support during the first year of bereavement. Volunteers provide non-clinical compassionate support either by phone or face-to-face visits.
